Mikal Mahdi was executed by a firing squad of three South Carolina Department of Corrections employees on April 11. An autopsy report shows that the bullets did not stop his heart and might have caused pain and suffering while he was still conscious. The autopsy notes two bullet wounds even though there were three shooters, and a forensic expert says the misfires likely caused "excruciating conscious pain and suffering."
